How to Partner with THD School Health

THD School Health is proud to be able to partner with any elementary school in Tulsa County!  In an effort to adequately meet the needs of our partnering schools and provide data to the Oklahoma State Department of Health we have developed a process. THD School Health requires Tulsa County elementary schools interested in partnering with us to complete a School Readiness Assessment along with having a district Memorandum of Understanding (MOU). The assessment allows us to gain an understanding of your school's climate and tailor our services accordingly.

2023-2024 School Year Partnership Dates
Assessment Dates: May 1st - August 18th (NOW CLOSED)
Planning Meetings: May 1st - September 1st

Step 1: Skills-based health education will be offered as a series of three in-person lessons. The series will consist of three skills-based health education lessons that will be taught once a month during a semester. There are stand-alone lessons that can be chosen that are not included in the series. All lessons will align with the National Health Education Standards and Oklahoma State Health Education Standards. Your school will select the preferred health education lesson(s); health education series, stand-alone movement and learning, stand-alone PE, bully prevention campaign. If you are interested in having THD School Health provide a professional development for your school, please review our Professional Development offerings.

Step 2: Review the PDF version of the assessment to ensure all necessary information has been gathered before beginning the online assessment. The information needed to complete the assessment will come from the 2022-2023 school year. If you are unable to answer a question, simply put N/A. We recommend collaborating with the principal, assistant principal, grade level team leads, PE teacher, nurse/health assistant, counselor, and community coordinator/parent liaison to collect the appropriate information and input for the assessment.

Step 3: Once all necessary information has been collected, complete and submit the THD School Health Readiness Assessment by selecting the black button at the bottom of the page. 

Step 4: Our team will reach out to schedule a planning meeting

Step 5: Meet with THD School Health staff to schedule programming for your school!

You are now partnered with the THD School Health Program for the 2023-2024 school year!

*Any assessment submitted after August 18, 2023 will NOT be accepted
*Planning meetings must be complete by September 1, 2023 for us to

partner with your school for the 2023-2024 school year
